Step 2: Identify Patients

Find SGLT-2 patients who meet program criteria.

Step 3: Clinical Assessment

Discuss therapeutic options with patients and obtain prescriber authorization.

Step 4: Patient Enrollment

Step 5: Provide Clinical Services

Complete clinical documentation in the following steps, to receive $32 compensation per eligible patient.

Step 6: Billing in Pharmacy Management System

Patient Criteria
